with the database you can just use the database in C# if you dont need the net, or else you will need to setup a mysql account on the net.
for the passing of the username to the next form, you could use settings.
right click on your project and go properties.
then go settings.
add a new string setting that is user set.
then then you have the login button on your form1, use this code:
Properties.Settings.Default.UserName = "My Username"; // You replace Username and My Username with the approprite things
then when accessing on the other form, for example if you use a label:
label1.Text = Properties.Settings.Default.UserName
